I need a PDF document that is functional.
IE. Someone can fill it out online save it and return it back.
Will include need for functional check boxes and text boxes.
Someone with a design eye. Forms need to look professional, clean with intuitive actions.
I have attached a document. It will be very similar to this. There will be some modifications (details to come).
Can you please resubmit your bid for your estimate on a form such as this one (with slight modifications)?
My initial total budget included the sum of several forms. I will post them as separate projects to give us both clarity on each job at hand.
I would like to work with one individual for all the forms and build a long-term relationship.

PDF medical Intake form that is digitally fill-able.
